He is survived by: his children, Nicole (Richard), Christopher (Corinna) and Jack; his grandchildren, Damian and Izabella; and his siblings, Diane (Mike), Jim (Lorena), Donna (Dave) and Heather (John). He is also survived by aunts, uncles, cousins, nieces, nephews & friends.
In lieu of flowers, the family is requesting donations be given to the Heart & Stroke Foundation or Steadman Hospice in Jack's honour.
