He is survived by: his parents, Harold Foulkes and Betty Foulkes; his sister Linda Moses (Jim); his nieces, Heather Cermak and Ashley Thompson; his great-nephews, Quinton, Mason Cermak and Michael Moses; his nephew Ares Moses; his aunt Reta; his uncle Don Simkin; his cousins, Wayne Simkin (Annette) and Brian Simkin (Debbie); and his friends, Dr. Victoria Dechert and Doug. He is also survived by his extended family and friends.
