Michael Kozak Obituary

With heavy hearts, we announce the death of Michael Kozak (Bolton, Ontario), born in Drumheller, Alberta, who passed away on January 1, 2019 at the age of 89. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He was predeceased by: his parents, William Kozak and Rose Kozak; and his wife Sophie. He is survived by: his son Michael David; his daughter-in-law Gaylor; his daughter Jessica; his brother Bill; his nieces, Jodi Harris, Zina Gagnon, Linda Downey and Deanna Zukowski; his nephew Dwayne Zukowski; and his cousins, Pauline Warwick and Gloria Meeder.

In lieu of flowers Michael has requested donations to be made to the Shriners Charities.
