He was predeceased by: his wife Margaret Duivenvoorde. He is survived by: his children, Nancy Perkins (late John), Theresa Matthews (Craig), Mary Irving (David), Ron Duivenvoorde, Brian Duivenvoorde (Barbara) and Mark Duivenvoorde (Ellen); and his sister Truus Warmerdam (late Peet). He is also survived by grandchildren and six great grandchildren.
Memorial donations to The Society of St. Vincent de Paul would be appreciated by the family.
