He is survived by: his children, Susan, Ed, Christopher and Teresa; his grandchildren, Michael (Kirsten), Christyn (Costa), Nicole (Derek) and Andrew (Jordan); his great grandchildren, Keigan, Mila, Rylan, Grayson and Nicholas; his sisters, Wardine (Claude) and Claudette (Don); and his sister-in-law Joanne (Al). He is also survived by Nieces and Nephews.

In lieu of flowers, donations to Headwaters Hospital in Orangeville would be appreciated and is Jack's wish.
