He was predeceased by: his father Donald Jack; his father-in-law Rodney Kaiser; and his nephew Nicholas Kaiser. He is survived by: his wife Rhonda (Kaiser); his children, Adrian (Megan) and Andrea (Stephen); his grandchildren, Soleil, Quinn and Dale Boudreau; his mother Connie Jack; his siblings, Ralph (Gwen), Myles (Wendy), Sandy (Peggy) and Darla Porter (Ricky); and his siblings-in-law, Rod (Shelly), Troy (Fern), Trent (Mamie), Nancy Humphries (Brian) and Carla Neilsen. He is also survived by many aunts, uncles, nieces, nephews, cousins and friends.

Memorial donations may be made to St. Martha's Regional Hospital Palliative Care Department or the Canadian Cancer Society.
