She was predeceased by: her husband Nestor Kozak; her parents, Barbara Hablichek and Jacob Hablichek; her siblings, Grace Parkes and John Hablichek; and her siblings-in-law, Margaret Hablichek, Tom Riley and Robert Parkes.

She is survived by: her nephew Cal Havlicek (Janice Cazakoff) of Durham; and her nieces, Jeanette Hoganson of Red Deer, Alberta and Peggy Stanley (Dave) of Langley, BC.
