He was predeceased by: his father William Dalton Lewis. He is survived by: his wife Jacqueline; his children, Jonathan Lewis and Dr Stefanie Lewis; his mother Wilma Jean Brown (Nictaux); his grandchildren, Brynne, Brock and Willa Lewis; and his siblings, Dave Lewis (Leona), Jeff Lewis (Christine), John Lewis (Nancy) and Tracy Ward (Mike). He is also survived by many nieces and nephews.
