He was predeceased by: his parent Carman Lewis Hazelwood. He is survived by: his mother Alice Marie (Spicer); his wife Nancy Llewellyn of South Greenwood; his sons, David Hazelwood (Gillian) of Kingston and Matthew Hazelwood (Tanya) of Torbrook Mines; his siblings, Sheila Phillips (Wayne) of Orleans, ON, Carol Dale of Kingston, Myrna Davis (Doug) of Nictaux, Tracey Hazelwood of Ottawa, ON, Michael Hazelwood (Louise) of Quebec, Fred Hazelwood (Darlene) of Kingston, Robert (Wendy Gallant) and Larry (Barbara); and his grandchildren, Zachery, Rachel, Nicholas and Anna.
