He was predeceased by: his parents, Henry Lively and Dot Lively (Laffin); and his siblings, Earl, Mae, Edith, Joan, Evelyn and Ethel.
He is survived by: his wife Judy (Ettinger); his granddaughter Darlene; his brother Murray (Judy); and his brother-in-law Wayne Waller. He is also survived by many nieces and nephews.
In lieu of flowers, memorial donations may be made to the Heart and Stroke Foundation or the Canadian Cancer Society.
