He was predeceased by: his parents, William H. Cook and Eva B. Cook (Reynolds); and his siblings, Herman A. Cook and Margaret Cook. He is survived by: his wife Barbara A. Cook (Cossett); his sons, David G. Cook (Anne), Michael Cook (Susan) and Mark K. Cook; his grandchildren, Sarah, Levi and Rachel; his nephew Peter W. Cook; and his nieces, Heidi Cook, Heather Cook and Venus.
