She was predeceased by: her parents, John Forward and Lucy Forward. She is survived by: her children, Kerry (Elizabeth), Kenneth (Jackie), Curtis and Douglas (Bonnie); her grandchildren, Kerri Ann (Tim), Kenneth (Lisa), Joshua (Justine), Alicia and Parker; her great grandchildren, Abbie, Kate, Tessa, Ellie, Rodney and Noah; and her siblings, Jim (Lilly) and Betty.

As expressions of sympathy, flowers are gratefully accepted or donations may be made to the Shirley M. Evely Memorial Fund.
