He is survived by: his son John; his sister-in-law Lori Cook; and his nieces and nephews, Carolyn Cook (Adam Snow), Jennifer Graham (John), Jason Cook (Charla), Sam Dmitruk-Cook (Ally) and Davin Dmitruk-Cook of Ontario.

In lieu of flowers, donations can be made to the charity of one's choice in Dave's memory.

“It’s been 26 years since I have seen my stepfather, Dave, which was when my mother, Sandra, passed away in 1998. He just entered my thoughts and I came across this obituary to find that he passed away just a week ago! He and my mother were very happily married and they are together again. It is a shame that we lost touch over the years. I hope the last years of his life were good and that he had good friends around him. Rest in peace.”
