She was predeceased by: her parents, Thomas Lass and Ruth Louise Lass (Miller); her husband Ernest Polchis II; and her sisters, Louise Stark and Lynn Lass.
She is survived by: her children, Jennifer Polchis (Vaughn Nicholas Jr.), Gena Polchis, Ernest Polchis III and Christopher Polchis; her grandchildren, Marissa, Malaya, Ace and Ernie; and her great grandchildren, Alayna, Titus and Kelsey.
