He was predeceased by: his wife Tina; his grandson Chad Kosak; his sisters, Helen Morrison and Carol Warkentin; and his parents, Mary Kosak and Michael Kosak.

He is survived by: his children, Peggy Fensom (Ernie), Donald Kosak (Sharon) and Susan Falkenberg (Gord); his grandchildren, Denise Hudson (Russ), Shelley Goshey (Tim Loewen), Trisha McGie (Shawn), Sean Kosak, Ashley Koley (Doug Bohay), Michael Koley, Randy Goshey, Dustin Brown, Samantha Hudson, Chelsy Johnson and Avery McGie; his great great grandchildren, Bryce Meeches and Scarlett Brown; and his brothers, John Kosak, Edward Kosak and Michael Kosak.
