He was predeceased by: his brother Joshua; his grandfathers, Peter Iwasiuk and Wallace Webb; and his aunts, Clarice Lavallee and Pauline Heckert.
He is survived by: his parents, Charlene and Jerry; his siblings, Jamie Monkman (Trevor), Michelle Polkowski (Adam) and Jerry Jr.; his uncle Roman Iwasiuk (Joyce); his nephews, Cory (Liz), Anthony (Bethany), Trevor Jr. (Riley), Liam and Aiden; his grandmother Sarah Webb; his cousins, Miranda, Michaela, Ryann Karatchuk, Frank McLeod and Madison; and his friend Lola. He is also survived by his many aunts, uncles, cousins and friends.
