He was predeceased by: his parents, Kjeld Herlevsen and Shirley Herlevsen; and his grandparents, Jim, Elva Perkes, Fred and Anne Herlevsen.
He is survived by: his wife Karen; his children, Joshua (Christine), Sara, Jessie (Jon), Cheylee (Matt) and Robert (Leroy); his sisters, Vicki (Kim) and Tracy (Doug); and his grandchildren, Hayden, Selena, Quinn, Kai, Kianna, Jack, Lily and Mason. He is also survived by numerous nephews and nieces.
