She was predeceased by: her parents, Andrew and Anastasia (Kostiuk); her children, Patricia and Dennis; her husband Adam Kozack; her brother-in-law Eugene Kozun; her brother Thomas Obedniak; her niece Evelyn Obedniak-Mauro (Evie); and her nephew Bruce Austin.
She is survived by: her siblings, Nicholas, Annie, Mary and Pauline; her son Terry Kozack (Shirley) of Melita; her granddaughter Shannon (Al Hay); her great grandchildren, Skylar and Dayton of Kamloops, BC; her sister Evelyn Kozun; her sister-in-law Sylvia Obedniak; and her niece Malveen Austin (family: Connie, Ted Speirs, Darlene, Bob Tanasychuk).
