He is survived by: his wife Susan Lewis; his children, Cindy Lewis (Cas), Shelley Hoffmann (Dave), Monty Lewis (Lida) and Nicole Lewis; his siblings, Craig, Linda, Rob and Howie; and his caregivers, District Hospital and Dr. Dykstra. He is also survived by his family and friends.
