She was predeceased by: her husband Arie Middelhoek; and her grandson Steven. She is survived by: her children, Jan Middelhoek, Henry Middelhoek, Hennie Boizard (Ernie), Liz Frederick (Doug) and Denise Ralph (Dale Chabaniuk). She is also survived by ten grandchildren; fifteen great grandchildren as well as numerous nieces, nephews, extended family and friends.
