He was predeceased by: his parents, Andrew Kozak and Pearl Kozak; his wife Diane; and his siblings, Marvin and Elsie Semeniuk (John).
He is survived by: his children, Sheri Belland (Yvon) of Morinville, Elden of Hamlin, Cindy Cherniwchan (Barry) of Bellis and Dean (Dawn) of Bellis; his grandchildren, Celina Cherniwchan, Kaila Nelson (TJ), Mackenzie Cherniwchan, Sabrina Belland (Josh), Jami Belland (Josh), Cassidy Kozak, Cody Kozak and Caleigh Kozak; his great grandchild Charlotte; his brother Nick (Donna) of Alliance; and his sister-in-law Carol Kozak of Edmonton.
