He was predeceased by: his parents, Peter Kiziak and Annie Kiziak; and his brother Orest Kiziak. He is survived by: his wife Katherine; and his siblings, Adeline Bielech and Paul Kiziak. He is also survived by numerous nieces, nephews, relatives and friends.
