He was predeceased by: his parents, Frantisek and Marie; his sister Helena; his grandson Giuseppe; his parent-in-law Big; his grandmother Anna; and his uncle Frank.
He is survived by: his former wife Angela Slavik; his children, Mary Hollinger (Mike), Anna La Montagna (Kevin), Frank Slavik (Vera), Giuseppina Slavik and Angela Slavik; and his close friends, Josef and Vreni Bellmont. He is also survived by his family in Czech and 15 grandchildren.
