She was predeceased by: her father Benoit Jacques. She is survived by: her husband Curtis Formanek; her daughters, Tamara Boe (Travis), Jessica Formanek and Cara Formanek; her grandchildren, Olivia, Harper, Indiana and Hayes; her mother Aline Jacques; and her siblings, Mariette Mahe (Pierre), Albert Jacques (Denise), Helen Chapdelaine (Yvon), Gaston Jacques and Doris Hurtubise (Bernie). She is also survived by numerous nieces, nephews, family and friends.
