He was predeceased by: his parents, Gregory and Wasylyna; his siblings, Mary Kozak and Harry; his brothers-in-law, John Kozak, Alex Kozak and Steve Fedoretz; his niece Debra Kozak; and his nephew Michael Kozak.
He is survived by: his wife Elizabeth; his sons, Lawrence, James (Heather) and Richard (Andrea); his grandchildren, Shayla, Cheyenne and Skylar Martin; and his siblings, Bill, Edward (Alice), Sandy (Nancy), Verna Kozak and Olga Fedoretz. He is also survived by numerous nieces & nephews.
