He was predeceased by: his parents, George and Wasylena; his siblings, Mike, Harry and Mary Kozak; his niece Debra Kozak; his nephews, Michael and Melvin Kozak; and his brothers-in-law, John Kozak, Alex Kozak and Steve Fedoretz.
He is survived by: his siblings, Edward (Alice), Sandy (Nancy), Verna Kozak and Olga Fedoretz. He is also survived by numerous nieces & nephews.
