He was predeceased by: his daughter Patricia Hays; his parents, Janet Jobagy and James Church; his stepfather John Jobagy; and his brother-in-law Rudy Harasym.

He is survived by: his wife Linda Church; his son Greg Church (Pamala); his grandchildren, Shelby Hays (Johnny), Andrew Church, Peyton Church, Blayze Bloomfield, Rhett Bloomfield and Marshall Bloomfield; and his stepsons, Jay Westwood (Sherry) of Jody Westwood, Matthew Westwood, Tanner Westwood, Isabella Westwood and Sophia Westwood.

In lieu of flowers, memorial tributes may be made directly to Foothills Hospital ICU, Calgary.
