He was predeceased by: his wife Anne; his parents, Nicholas Polischuk and Parascavia Polischuk; and his siblings, Anne Zyla (John), Mary Slepanki (Harry), Lloyd (Jeanette), Paul (Rose) and Olga Polischuk.
He is survived by: his son Peter (Cindy); his grandchildren, Jordan (Mikayla) and Jaden (Kaitlyn); and his close friend Patricia Tompkins.
