She was predeceased by: her husband Nick; her parents, Hafia Lyzanchuk (Lychak) and Frederick Lyzanchuk; and her brothers, John Kostiuk and Lychak.
She is survived by: her children, Jason Zayac (Carmen) of Vermilion, Laurie Fletcher (Ken) of Calgary, Valerie Sadowsky (Bob) of St. Albert, Candace Phillips (Raymond) of Beauvallon, Carol Brown (Jim) of Streamstown, Donna Campbell (Jim) of Edmonton, Margie Boyko (Delmer) of Elk Point, Shirley Johnson (Bob) of Hudson, Ohio and Ervin Zayak of Derwent; her grandchildren, Casey, Ainsley, Emery Zayac, Nicholas Fletcher (Sarah), Mackenzie Fletcher, Bailey Bow (Landon), Jordan Sadowsky, Karla Phillips, Murray Phillips (Kaila), Darren Phillips (Adele), Tyson Phillips, Julie Phillips, Justin Brown (Coralee), Jen Brooks (Ryan), Mitch Campbell (Jodie), Ashley Boyko, Lindsey Boyko (Tyler), Camille Johnson, Grace McClurg (Graham) and JoAnn Barlow (Colton); and her great grandchildren, Ethan, Carter Berdan, Mason, Carson, Payton Phillips, Mia, Trey Brooks, Ryan, Austin, Treston, Hailey Campbell, Noah, Lily, Adaline Boyko, Logan, Maisie Cronk, Seth Bourassa and Leona Barlow. She is also survived by numerous nieces & nephews.
