He is survived by: his wife Lorraine; his children, Michael (Jennifer) and Cathy Douglas (Kevin); his grandchildren, Jake, Kai, Luke and Brooklyn; and his sister Maureen Perepelkin (John). He is also survived by extended family and many friends.
In lieu of flowers, donations may be made in Jack's Memory to Pilgrim Hospice Society (The Roozen Family Hospice Centre).
