He was predeceased by: his father Herbert Kulak. He is survived by: his son Herb (Rose Kulak); his mother Rose; his siblings, Elaine (Gary) and Randy (Juanita); and his nieces and nephews, Cassandra, Jamie, Lyndon, Hollie, Brandon and Curtis. He is also survived by 15 great nieces and nephews.
