He is survived by: his son Jon Hoffman (Danielle Ostifichuk); his siblings, Melody Maher, Lyle Hoffman and Leonard Hoffman (Lynn); his mother-in-law Betty Jacobson; and his sister-in-law Barb Jacobson.
