He was predeceased by: his father Lawrence Wayne Phillips; and his nephew Kyle Phillips. He is survived by: his mother Peggy Ann Phillips (Long); his wife Paula Rae Phillips (Okulanis) of Moundsville; his sons, Joshua P. Phillips of McMechen, WV and Jesse M. Phillips of Moundsville; and his siblings, Joan Kennedy of Fredrickstown, PA, Don Phillips (Susan) of Georgetown, OH and Matt Phillips (Casey) of Stafford, PA. He is also survived by several nieces and nephews, one great-niece, one great-nephew.
