He was predeceased by: his parents, James Smallwood and Jesse Smallwood; his wife Shirley Smallwood; his grandson Wesley Smallwood; and his siblings, Denny Smallwood, James "Bud" Smallwood Jr., Larry Smallwood, Danny Smallwood and Judy Bissett.
He is survived by: his children, Donald Smallwood Jr., Steven Smallwood, Jeffrey Smallwood and Sandra Smallwood (Meredith McCue); his grandchildren, Casey Smallwood, Callie Smallwood, Zachary Smallwood, Michael Smallwood, John Smallwood and Stephanie Rico; and his siblings, Bonnie McCabe, Carol Forman, Eddie Smallwood and Roy Smallwood.
