She was predeceased by: her parents, George "Jack" and Marjorie Cook. She is survived by: her son Patrick Alan Hazelett (Amy); and her siblings, Donna Moore (Tom) of Hurricane, Jackie O'Brien (Dave) of Hurricane and Mark Cook (Penny) of Nitro. She is also survived by nieces and nephews.
