He is survived by: his wife Sandra Carol Fazalare (Brosius); his children, Fred Fazalare (Sandra) of Shinnston, Gary Fazalare (Amie) of Shinnston and Lisa Fazalare of Fairmont; his grandchildren, Freddy Fazalare, Jessica of Mannington, Bethany King (Burnell), Brandon and Cory Fazalare of Shinnston; and his brothers-in-law, James Brosius (Carol) of Goshen, Indiana, David Brosius (Margaret) of Erie, John Brosius (Nancy Brosius) and Bridgeport. He is also survived by 9 great-grandchildren.
