He was predeceased by: his father LeRoy Hooker; his grandparents, Harold Smith (Clara) and Elva Hooker; his godfather Bill Smith; his aunts and uncles, Larry Hooker, Harold Smith (Jackie), Ken Smith, Jerry Smith, Irvin Smith, Joyce Hooker, Betty Erickson and Marcella Long; and his godmother Yvonne Fosdick.

He is survived by: his mother Judie Hooker (Smith); his stepfather Rick Ellett of Minocqua; his children, Max Hooker of Wisconsin Dells, Odin Hooker of Wisconsin Dells and Luke Hooker of Appleton; his girlfriend Lisa LaRue of Ironton; and his aunt Avis DeMars Roessler of Baraboo. He is also survived by many cousins along with other relatives and friends.

Memorials in memory of Terry preferred in lieu of flowers.
