He was predeceased by: his mother Kathryn Jeanette Lewis (Rogers); his sisters, Carole Louise Lewis and Elizabeth Jane Davies; his siblings-in-law, Paul Oliver Mittelstaedt, Dan Goss and Carolyn Goss; and his nieces, Stacy Helen Mittelstaedt and Ella Shine Lauritzen-Davies.
He is survived by: his father Carroll Edgar Lewis; his pet Freckles; his child Meredith Sarah Lewis; his former spouses, Kip, Colin, Cara, Katie and Casey; his wife Mona Goss; his children, Christopher "Kip" Lewis (Diane), Colin Lewis (Yolanda), Cara Lewis, Katie Pulda (Bob) and Casey Lewis; and his siblings, Tim Lewis (Bev), Mary Pohl (Dave), Mimi Meighan (Pat), Patsy Mittelstaedt and Michael Lewis (Solange).
The family will choose an organization later and make a donation in Tom's name.
