Jeffrey T. Blazek Obituary

It is with great sadness that we announce the death of Jeffrey T. Blazek of Poynette, Wisconsin, born in Portage, Wisconsin, who passed away on December 24, 2021, at the age of 51, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He was predeceased by: his father Edward Blazek. He is survived by: his mother Elizabeth Blazek (Smith); his daughters, Lilly and Lillian Rae Clemens; his siblings, Ed Blazek (Dawn), Ann Angell (Tom Heaps) and Pamela Blazek; his nieces and nephews, Eddie Blazek, Beth Angell-Satterfield (Randy), Ashley Angell and Jacob Walter (Jesica); his great-nieces and great-nephews, Brayden Alves, Jordan Walter, Angel Alves and Isla Satterfield; and his close friend Bandit. He is also survived by aunts, uncles, cousins, other near relatives, and friends.

Masks are recommended for attendance at Jeff's services.

In lieu of flowers, memorials may be made to the family in Jeff's honor.
