He was predeceased by: his parents, Antone Blazek and Lucille Blazek (Phillips); his siblings, Patsy Swenty, Connie Stodola (Ed), Delores Rice (Gene), Shirley Oleck (John) and William "Bill" Blazek; and his brothers-in-law, Leroy Swenty, Tony Decleene and LeRoy Swim.
He is survived by: his wife Vivian Pelishek; his sons, Sam Blazek of Spruce and Eric Blazek (Paula) of Lena; his grandchildren, Elizabeth Saari (Nick), Holly Marquardt (Tyler), Morgan Blazek (Mason) and Luke Blazek; his great grandchild Levi Saari; his brother Antone "Tony" Jr.; and his siblings-in-law, Rose Decleene, Orianne Swim, Walter Pelishek (Cleo), Henry and Martha Pelishek. He is also survived by many nieces and nephews.
