He was predeceased by : his parents, Elizabeth Ax (Kraak) and Halbe Ax; and his daughter Jamey Ax. He is survived by : his wife Mary; his sons, David Ax of Harrisburg, PA and Michael Ax of Asheville, NC; his sister Willemina Van Santen (Abel); his niece Annelise Van Santen; and his nephew Halbe Van Santen.
