He was predeceased by: his parents, Jerome "Jerry" and Elizabeth Berkholtz (MacPhail); and his aunts, Eleanor Holub (Don) and Grace Reese (George). He is survived by: his girlfriend Christy Polzin; his brother Charles "Tanner" Berkholtz (Tracy Doherty); his parents-in-law, Ken and Connie Polzin; his sisters-in-law, Charity Rasmussen (Matt), Chasity Polzin and Casey Konrad (Ryan); and his nieces and nephews, Wyatt, Waylon, Ana, Konnor, Carly, Keenan and Chandler.
