He was predeceased by: his father Chuck Lewis. He is survived by: his mother Jeann Lewis (Collins); his sister Brenda Lewis (Kimberly); his fiancée Heidi Nelson; his niece Taylor Lewis (Elroy); his great-niece Gemma Lewis (Elroy); his great-nephews, Jeremiah Perrigo and Jax Perrigo; and his stepchildren, Jennifer, Jackie and Justin. He is also survived by other relatives and friends.
