He is survived by: his parents, Jack and Diane; his grandmother Doris; his brothers, Jack Jr. (Angela), Steven (Rochelle) and Thomas (Rebecca); his nieces and nephews, Riley and Anastacia; his aunt Jan; his uncle Gerald; his cousins, Mark, Jeremiah, Joshua and Sarah; and his girlfriend Janice Radke. He is also survived by his family as well as has many friends.
