He was predeceased by: his parents, Forrest Stephen Cook and Myrtle E. Cook; and his siblings, Marcella Schwanz, Arlene Gonzagowski, Franklin Cook and Donald Cook.
He is survived by: his wife Karen Cook of Paul's Lutheran Church; his children, Vicki Hornbeck (Joe) and Randy Cook (Patti); his grandchildren, Ashley Sanner (Nathan) and Elizabeth Stewart (Josh); and his siblings, Betty Koch, Mary Schell and Dean Cook. He is also survived by many nieces, nephews, and friends.
