He was predeceased by: his parents, Frank Costanza and Linda Costanza (Dale); his wife Cathy; his granddaughter Kari; and his great grandchild Franklin.

He is survived by: his children, Todd (Michelle), Jode (Rich) and Kimberlei (Jeff); his sister Linda (David); his close friend Don; his brothers-in-law, Jim (Kathe) and Ron (Theresa); and his aunt Francie. He is also survived by cousins, nieces & nephews, friends.
