He was predeceased by: his father Richard Allen. He is survived by: his mother Donna Leigh Clark; his sisters, Debbie Klinefelter (Steve) and Lisa Butler (Doug); his niece Kimberly La Fuente; and his nephews, Jeffrey Proctor and Tyler Meyer. He is also survived by two great nieces, a great nephew, aunts, uncles and cousins.
