He is survived by: his former wife Janice Haynes; his wife Rosalie Tibbs; his sister Sharon Kubik of Astoria; his sons, Mike Kubik (Michelle) of Mesa, AZ, Kenneth Kubik of Sitka, AK, Donald Kubik of Sitka, AK and Christopher Proffer of Kauai, HI; and his grandchildren, Kelsi, Devin, Jade and Alan. He is also survived by many nieces and nephews.
