He was predeceased by: his mother Wes Clements; his wife Patricia Joan "Jo" Clark; his children, Cynthia Jane and Eric Eugene; his brother James; and his son Forrest Michael.
He is survived by: his daughter Tammy Yeakey; and his daughter-in-law Kris Petersen.
