He was predeceased by: his parents, John E. Provasnik and Irene Hagan. He is survived by: his wife Sharon McNamee Rockwell; his sons, Kevin Provasnik, Gregory Provasnik, Daniel Provasnik and David Provasnik; his stepdaughter Kathleen Rockwell (Katie); and his caregiver Irv.
